Taking Control of Your Health is the ongoing programcoordinated by MBGH, in cooperation with the Illinois Pharmacists Associationand is supported by Novo Nordisk, Inc. “This program hasenabled us to provide our employees that have diabetes with additional toolsand support to help them actively manage their condition as well as incentivesfor them to take action.”MBGH was one of more than 30 employer groups throughout the country thatparticipated in the Diabetes Ten City Challenge, a national employer-baseddiabetes self-management program conducted by the American PharmacistsAssociation Foundation through its HealthMapRx(TM) program, with support fromGlaxoSmithKline. MBGH partners with the Illinois PharmacistAssociation to train pharmacists in patient education, monitoring of clinicalconditions and motivation according to guidelines of the American DiabetesAssociation. The objective is to enable patients to reach individual goals formedication compliance, fitness and weight management. The first-year results looked at 86 participants who received benefits fromfour Chicago-area based employers: the City of Naperville, Jewish Federationof Metropolitan Chicago, Hospira and Pactiv Corporation.”Not only did we realize an improvement in clinical indicators and costsavings, but we were pleased to see a high level of satisfaction from ourparticipating employees regarding their overall diabetes care,” said JudithHearn, Health and Welfare manager for Pactiv Corporation. Average total health care costs for participants declined by almost$625.The TCYH program, which now has over 200 participating patients, is open tocovered employees, dependents and retirees from employer members of theMidwest Business Group on Health. There are three unique elements of theprogram: 1) The employer reduces or waives co-pays for diabetic drugs andsupplies; 2) The patient signs an agreement that commits them to education andface-to-face coaching on diabetes self-management in return for the reduceddrug costs; and 3) Pharmacists are trained as diabetes coaches to assistpatients on a regular basis.
